For the UK Skilled Worker visa (including Health & Care Worker route), you need to prove English at CEFR B1 minimum — which maps to IELTS 4.0 in each section (Life Skills B1 or IELTS for UKVI).

Accepted English Tests for Skilled Worker

UKVI accepts these Secure English Language Tests (SELTs):

Life Skills is shortest and cheapest if B1 is all you need.

Health & Care Worker Visa Specifics

The Skilled Worker visa has a special Health and Care Worker subtype. Language requirement:

  • Same B1 level as standard Skilled Worker
  • But your employer (NHS trust or care home) may require higher IELTS for NMC/GMC professional registration (typically IELTS Academic 7.0 each for nursing — see our AHPRA/NMC guides)

So two separate levels:

  • Visa level: B1 (IELTS 4.0 each or Life Skills pass)
  • Professional registration level: typically IELTS 7.0 each (nursing) or OET

You need the higher of the two to practice as a nurse or doctor.

Who's Exempt from UK Skilled Worker English

You don't need to prove English if:

  • You're a national of a majority English-speaking country (USA, Canada, Australia, NZ, Ireland, Caribbean, etc.)
  • You have a degree taught in English from a recognized institution
  • You've previously scored B1+ on an accepted test within 2 years

Employer must attach evidence to Certificate of Sponsorship (CoS).

IELTS Life Skills vs IELTS Academic

IELTS Life Skills B1:

  • Tests Speaking + Listening only (no reading/writing)
  • 18-minute test with another candidate + examiner
  • Pass or fail
  • USD 170–200
  • Faster, cheaper, simpler for visa-only purposes

IELTS for UKVI Academic:

  • Tests all 4 skills
  • 2h 45min test
  • Band score 0–9
  • USD 240–260
  • Use this if you also need score for professional registration

3 Common Mistakes

  1. Taking Life Skills when you also need NMC registration — waste of money. Take IELTS Academic so you have a score for both.
  2. Taking expired tests — scores valid 2 years at visa submission (some employers impose shorter internal limits).
  3. Taking non-UKVI IELTS — the cheaper regular IELTS won't work for visas; UKVI-accredited only.
